Content of Report :
                                                                                                                                        
INTRODUCTION
This report is a summary of the content and outcomes of a training program held in Perth (Western Australia) in May 2012.
The training program was organized by Patricia O’Sullivan Humanitarian Project, in cooperation with Western Australia Government and East Java Government.
The aim of this program was to highlight the urgency of early detection and early intervention for deaf children, in order to allow trainees for exchanging experiences in making the issue a policy priority and developing, implementing Resources Center Program for children with special needs.

1.        Background       
Sound Hearing 2030 was applied, early detection and intervention program that was held for prevention on deaf mute and dealyed speech among children. Based on the increasingly high levels of public awareness of the importance of health and the increasing number of incidents of hearing loss, especially with the demands of some company or profession who is performing auditory testing, it would require the ability to accomplish diagnosis of hearing loss accurately and quickly. The existence of audiometric services is important, as part of an effort to improve health care in Surabaya, Gresik, Sidoarjo and Mojokerto.

1.1      Purpose of the report
The purpose of this report is to describe the result of this training program. The report provides:

2.        The training
2.1  Objectives of the training  
The main objective of the training was to provide some basic hearing science, be able to conduct some basic hearing tests for children, adults and older children, and have some knowledge about the causes and possible treatments for hearing loss.
The specific objectives were to improve human resources (audiologist & speech therapist)
The Audiology Trainees will be able to:
1) Provide a basic hearing test for children from birth. This will mean that I will need to teach them
a.       Some behavioural techniques for testing babies (behavioural observation audiometry), toddlers (visual response audiometry) and young children (play audiometry).
b.      Testing and interpreting tympanometry
c.       Testing and interpreting some basic speech tests – we can perhaps develop one with the trainees while they are in Perth. For example we use the Kendall Toy test for children aged from 3-6 years and it is a closed set 10 item monosyllabic word test using toy items.
d.      Using otoacoustic emissions as a screening tool
e.       take histories from parents before the testing, and provide feedback to parents on the results.
f.       Knowing when and who to refer children to given the results of the tests.
g.      Understanding what a child hears based on the severity of their hearing loss and how this impacts on their speech and language development

2) Manage trouble shooting of hearing aids that are not working

3) Understanding what a hearing aid is, what it can do and what it can’t do (especially in relation to speech and language development)
a.       Understand how a hearing aid is selected based on the severity of a hearing loss
b.      Understand why a hearing aid does NOT give a child normal hearing, and have an appreciation of the type of hearing a child has given their loss and their hearing aids.

4) Have a basic understanding of cochlear implants and their limitations
5) Provide information to teachers and other professionals about managing hearing-impaired children in their classroom, school, other centres.

6) Have an understanding of other disabilities and how these might impact on a child who has a hearing loss.

The speech, language and communication trainees will be able to:
1) Understand speech and language development in a child with normal hearing

2) Understand how the severity of a hearing loss impacts on a child’s speech and language development (what sounds they can hear/what sounds they cannot hear both with and without hearing aids)

3) Understand that a child needs to have adequate amplification in order to undertake acquire “oral” communication.

4) What to do if a child does not have hearing aids?

5) Understand and be able to use some auditory-verbal teaching techniques

6) Understand why a signing language is needed for some children.

2.2     Participants of the training 
The training was attended by participants who came from different background,city/region such as Surabaya, Sidoarjo, Gresik, Mojokerto.The Government of East Java province will built a place called Resources Center for Hearing and Language Development as soon as possible.This training following by :
1.      Doctor (ENT doctor from dr.Sutomo Hospital Surabaya)
2.      Nurse (from Airlangga University Hospital, Social Agency, Public Health Services from Sidoarjo and Gresik)
3.      Teachers of Deaf children ( from Karya Mulya Surabaya  and Dharma Wanita Sidoarjo School)


2.3     Resources for the training
Resources for the training are:
1.      Experienced and certified trainers
2.      Complete equipments
3.      Comprehensive eductional methods
4.      Right funds

3.        Content of the training
The lecture represented at the training are at different stages in topics about:
3.1     Basic in Hearing Science
Basic in hearing science describe about the anatomy and physiology of hearing, This training also give us knowledge Introduction of acoustic including about Pure tone, complect tones, Frequency and Pitch, Intensity and Loudness

3.2     Kinds of Hearing Test in Children
These basic hearing test for children from birth consist of
3.2.1   using otoacoustic emissions as a screening tool
3.2.2   some behavioural techniques for testing babies (behavioural observation audiometry), toddlers (visual response audiometry) and young children (play audiometry).
3.2.3   Tympanometry / Reflex Impedance
3.2.4   basic speech audiometry tests
3.2.5   using otoacoustic emissions as a screening tool

3.4     General Paediatric Hearing Development
General Paediatric Hearing Development begin from Embriology, include the nerves pathway and the tiny bone ( Malleus,Incus and Stapes), the cochlea.The Auditory Embryopathies  caused by TORCH infection  have been associated with varying degree of hearing loss in children

3.5     Children Speech and Language Development
Working with deaf children who are developing speech and language, It is very important to understand early language development in normally hearing babies and young children.The right scalfholding is very useful than  just correcting too much.
The main points  of using spoken or sign language are:
1.        Always use hearing device
2.        Use daily routines : talk about what are you doing and what for you doing that
3.        Respond positively to communication attempts
4.        Get in close, use eye contact
5.        Give immediate into respons to baby’s sound
6.        Waiting for baby respond and encourage them talking or signing
7.        Draw attention to environmental sounds
8.        Expand and Extend Language
9.        Establish joint attention

3.6     Kinds of Speech Test
The kinds of Speech Test are :
1.        The Ling Six Sound test  
2.        Oral motor Evaluation
3.        Voice Evaluation
4.        WIIG-SEMEL Test of Linguistic concept


4.        Summary
The main objective of the training was to provide some basic hearing science, be able to conduct some basic hearing tests for children, adults and older children, and have some knowledge about the causes and possible treatments for hearing loss.


5.        Suggestion
1.         Need to increase more and update knowledge, advanced training or specialized audiological and speech education, over  the 3 weeks of training.
2.          Sustainable program, the trainees are expected to hold meetings periodically to upgrade and solve problems together.
